Here is the answer. If the average adult produces $20,000 of output per year, the amount of global output that is  lost annually as a result of adult deaths from secondhand smoke is 48 billion dollars. Consider this:
600,000 total deaths 
165,000 children 
435,000 adults 
435,00 times $110,00 output per year = $47,859,000,000 or $48 billion.
